Instructions for M Sc Allied and Health Care Professional Courses Counselling Session 2025-26
The Counselling will be held on 20/08/2025 (Wednesday) in Sri Guru Ram Das University of Health Sciences, Sri Amritsar.
Reporting Time: 10:00 AM
The candidate must bring all the following documents at the time of counselling:

Printed copy of their Application form. |

Original matriculation examination certificate indicating the date of birth. |
|
c) |
Original matriculation examination mark sheet indicating “pass” in the subject of Punjabi of that standard; (For candidate from Punjab State only). |

d) |
Original marks sheet certificate of 10+2 or equivalent examination/ Printed copy of 10+2 result. |
|
e) |
Detail Marks Card and Degree of qualifying exam where applicable. |

f) |
Original Character Certificate from the school / college last attended. |
|
g) |
Original Migration Certificate from last attended Board. |
|
h) |
Four recent passport size photographs with name and father’s name written on the back side. |

i) |
Gap Year Affidavit (if applicable). |
|
j) |
Copy of Aadhar Card. |
|
k) |
Recent Domicile Certificate/ Bonafide Resident of Certificate. |

Affidavit (Sikh Minority quota Candidates) |

Original Caste Certificate for SC/BC Candidates |

Note:
- Candidate must bring a clear bag with them.
- Demand Draft of *Rs. 36,500/- M.Sc Medical, in the name of Sri Guru Ram Das Chrt. Hosp. Trust, University A/c payable at Amritsar.
- Demand Draft of *Rs. 41,500/- M.Sc Genetic, in the name of Sri Guru Ram Das Chrt. Hosp. Trust, University A/c payable at Amritsar
